List all members of your household including age and gender, including yourself (we ask because some dogs have some issues with people of certain ages or genders):
How many hours a day will the dog be left alone?
Where will the dog(s) be kept when you are not home?
Does anyone in your household have any known allergies to dogs?
What animals do you currently have? Size (lb): Age(s): Spayed/Neutered?
Would you foster a dog with significant medical/special needs pet? (blind, deaf, needing physical rehab or med such as eye drops, etc)
Would you foster a dog that is known to have a bite history? (many times this is out of fear and we will advise you on details with case)
If you foster a dog who needs to be spayed or neutered, would you be willing to drop it off/pick it up at the vet?
in Manhattan (usually at 211 E 5th St, NY NY (Cooper Sq) or 326 East 110th Street, NY NY)If your dog needs other veterinary care would you be willing to take it to a vet visit? (Vet visits are typically weekdays, 10-6, at 5th st location. We also have other vets we occasionally use but all vets visits must be with one of our vets.)
Will you be able to keep the dog until placement as is expected (varies but usually 1-4 weeks)?
If you not able to commit to keeping dog until placement at this point and are instead applying to be short term foster placement please specify time line (e.g. 48 hours, one week, etc).
